Badge Award Photo Consent Form - CCAA | Charity Supporting Children With JIA

If you would like to send us a photo of your child wearing their badge/lanyard we would be delighted to share it on our social media and Website to encourage others to apply for awards too!

You can either upload your photo below or send the photo to awards@ccaa.org.uk.

We may use these photos for raising awareness of JIA, meaning that they may appear on our website, social media platforms (Facebook/Instagram/TikTok) and on our printed publications and presentations.

CCAA will only use the photo for the purposes stated above.

You may withdraw consent at any time by emailing awards@ccaa.org.uk, please note if the photo has been used online or in printed publications it will be very difficult to recall once it has been published.
