Events Consent Form - CCAA | Charity Supporting Children With JIA

The CCAA local groups are a network of friendly groups run by volunteer parent reps that, amongst other things, arrange regular meet-ups for the whole family in their local area.

While the type of meet-up may vary, we would like to ensure that anyone participating in these events is clear about their responsibilities, as these events are unsupervised.

Parents or carers are responsible for their own Health & Safety and that of any children they bring with them on the day and must supervise them appropriately at all times. Any activities undertaken are in line with the venue’s requirements and relevant Risk Assessments.

Please gain permission from other participants (children, parents/carers) before sharing photographs or videos on Social Media that include them.
